electronic records describing your health history, symptoms, examination and test
results, diagnosis, treatment and plans for future care or treatment.

Inc. is not required to agree to any restrictions requested.
You are allowed to revoke this consent in writing, except to the extent that
the organization has already taken action in reliance thereon.
Please note, if you refuse to sign this consent or revoke this consent, HeartCare,
Inc. may refuse to treat you as permitted by Section 14.506 of the Code of Federal
Regulations.
HeartCare, Inc. reserves the right
to change their notice and practices and prior to implementation, in accordance
with Section 164.520 of the Code of Federal Regulations.
Should HeartCare, Inc. change their notice, they will send a copy of any
revised notice to the address you provide.
