Privacy Statement


As part of your healthcare, Heartcare, Inc. originates and maintains paper and/or electronic records describing your health history, symptoms, examination and test results, diagnosis, treatment and plans for future care or treatment.  This information serves as:

  • A basis for planning your care and treatment.
  • A means of communication among the many health professionals who contribute to your care.
  • A source of information for applying your diagnosis and surgical information to your bill.
  • A means by which a third-party payer can verify that services billed were actually provided.
  • A tool for routine healthcare operations such as assessing quality and reviewing the competence of healthcare professionals.

 

Prior to your visit, you will be given a copy of the “Notice of Information Practices”. It will provide a more complete description of uses and disclosures of your health information.  You will be asked to sign a patient consent about the Use and Disclosure of Health Information for Treatment, Payment or Healthcare operations. You will be fully aware of your rights and privileges such as:

  • The right to review the “Notice” prior to signing the consent.
  • The right to object to the use of your health information for directory purposes.
  • The right to request restrictions as to how your health information may be used or disclosed to carry out treatment, payment or health care operations.

 

It is important to know, that Heartcare, Inc. is not required to agree to any restrictions requested.  You are allowed to revoke this consent in writing, except to the extent that the organization has already taken action in reliance thereon.  Please note, if you refuse to sign this consent or revoke this consent, HeartCare, Inc. may refuse to treat you as permitted by Section 14.506 of the Code of Federal Regulations.

 

HeartCare, Inc. reserves the right to change their notice and practices and prior to implementation, in accordance with Section 164.520 of the Code of Federal Regulations.  Should HeartCare, Inc. change their notice, they will send a copy of any revised notice to the address you provide.
