become better prepared and informed. Please feel free to call any HeartCare, Inc. location if your questions
are not answered here.
In order to make your visit as helpful as possible we ask that you follow the tips below:
1. Bring a list of questions and concerns you would like to discuss with your physician
to your visits. List them in the order of most significant.
2. Bring your current prescription and non prescription medication bottles with you
to your visit. Also make a list of all other medications you have taken in
the last 3 months.
3. Utilize our “patient education” section of the website to better inform you of medical
topics and conditions that you may want to discuss.
4. If you are unsure of your insurance coverage or have billing questions prior to
your visit, please call your insurance carrier or our billing department at any
of the office numbers listed. Sometimes office visits, procedures, tests and
other necessary treatments may not be covered by your plan. Please note, any
balances not covered by your insurance will be your responsibility. Co-payments
are due at time of your visit. Financial assistance is available if you
qualify. Please contact our billing department for more information.
In case of an emergency, please seek assistance at nearest emergency hospital or
call 911.
